ABSTRACT

Objetivo: investigar o impacto da deformidade bucal oncológica na comunicação oral e na qualidade de vida de adultos e idosos. Métodos: estudo observacional descritivo, transversal, com abordagem mista. Amostra de 41 pacientes, submetidos à cirurgia para retirada do câncer bucal, resultando em mutilação e inteligibilidade verbal. Instrumentos: Questionário sociodemográfico; Índice de Desvantagem Vocal; Questionário Qualidade de Vida da Universidade de Washington; Roteiro Semiestruturado para análise no software Iramuteq®.Resultado: a mutilação predominou em região de palato. Avaliação vocal mostrou o sexo masculino com pior qualidade de vida, expressivamente escala Social, enquanto, o feminino, escala Física. A análise lexical mostrou necessidades, frustrações e expectativas. Conclusão: o estudo revelou a vergonha, o medo, o isolamento social e, ao mesmo tempo, a esperança de voltar a, pelo menos, poder falar e ser compreendido pelas outras pessoas através do uso da prótese


Objective: to investigate the impact of oral cancer deformity on oral communication and the quality of life of adults and the elderly. Methods: descriptive, cross-sectional observational study with a mixed approach. Sample of 41 patients undergoing surgery to remove oral cancer, resulting in mutilation and verbal intelligibility. Instruments: Sociodemographic questionnaire; Voice Handicap Index; University of Washington Quality of Life Questionnaire; Semi-structured script for analysis in the Iramuteq® software. Result: mutilation predominated in the palate region. Vocal assessment showed the male sex with worse quality of life, significantly on the Social scale, while the female, Physical scale. Lexical analysis showed needs, frustrations and expectations. Conclusion:the study revealed shame, fear, social isolation and, at the same time, the hope of returning to at least being able to talk and be understood by other people through the use of the prosthesis


Objetivo: investigar el impacto de la deformidad del cáncer oral en la comunicación oral y la calidad de vida de adultos y ancianos. Métodos: estudio observacional descriptivo, transversal con enfoque mixto. Muestra de 41 pacientes sometidos a cirugía para extirpar el cáncer oral, lo que resulta en mutilación e inteligibilidad verbal. Instrumentos: cuestionario sociodemográfico; Índice de discapacidad de voz; Cuestionario de calidad de vida de la Universidad de Washington; Guión semiestructurado para análisis en el software Iramuteq®. Resultado: la mutilación predominó en la región del paladar. La evaluación vocal mostró el sexo masculino con peor calidad de vida, significativamente en la escala social, mientras que la hembra, la escala física. El análisis léxico mostró necesidades, frustraciones y expectativas. Conclusión: el estudio reveló vergüenza, miedo, aislamiento social y, al mismo tiempo, la esperanza de volver al menos a poder hablar y ser entendido por otras personas mediante el uso de la prótesis

ABSTRACT

RESUMEN El linfoma de Burkitt, se trata de un subtipo poco frecuente del linfoma no Hodgkin, con elevada frecuencia en aquellos pacientes con sida. La hepatoesplenomegalia es un signo clínico de gran importancia para el diagnóstico oportuno de algunas patologías; entre los mecanismos de formación de la hepatoesplenomegalia se encuentra la infiltración celular, ocasionada por la migración de células tumorales. Se presenta por inflamaciones debido a la presencia de infecciones por virus o bacterias las cuales son muy comunes en pacientes con sida. Se presentó un caso de un paciente masculino de 4 años, diagnosticado con VIH positivo, con la configuración correspondiente de criterios clínicos en clasificación C para sida. El cual desarrolló a nivel de cavidad oral un Burkitt primario, que se acompañó de hepatoesplenomegalia. Se pretendió describir la relación y el comportamiento de este tipo de linfoma con la hepatoesplenomegalia, así como la repercusión a nivel del sistema estomatognático, a nivel sistémico y el plan de tratamiento. Por el cuadro clínico e inmunológico del paciente estudiado, se planteó un pronóstico reservado por presentar un cuadro clínico infrecuente, en el que se observó Burkitt; tanto a nivel del sistema estomatognático como a nivel abdominal. Se hizo necesario realizar un diagnóstico oportuno y certero para iniciar el tratamiento a tiempo, se comenzó inmediatamente con tratamiento (AU).


ABSTRACT Burkitt lymphoma (BL) is a rare subtype of non-Hodgkin lymphoma, with high frequency in those patients with AIDS. Hepatosplenomegaly is a clinical sign of great importance for the timely diagnosis of some pathologies; cellular infiltration is found among the mechanisms of hepatosplenomegaly formation; it is caused by the migration of tumor cells. It emerges by inflammations due to the presence of infections by virus or bacteria which are very common in patients with AIDS. The authors present the case of a male patient, aged 4 years, with a positive HIV diagnosis, and the correspondent configuration of clinical criteria in C classification for AIDS, who developed a primary Burkitt lymphoma at the level of oral cavity We present the case of a 4-year-old male patient diagnosed with HIV positive, with the corresponding configuration of clinical criteria in classification C for AIDS; who developed a primary LB at the oral cavity level that was accompanied by hepatosplenomegaly. The authors pretended to describe the relation and behavior of this kind of lymphoma with hepatosplenomegaly, and also the repercussion at the stomatognathic level, at the systemic level and the treatment plan. Due to the clinical and immunological characteristics of the studied patient a reserved prognosis was given because of presenting infrequent clinical characteristics in which a Burkitt was observed both, at the stomatognathic and at the abdominal level. It was necessary to make an opportune and accurate diagnosis to begin the treatment on time (AU).

ABSTRACT

Introdução: A osteorradionecrose acomete de 1% a 6% dos pacientes submetidos à radioterapia e é considerada a complicação oral mais grave advinda dessa modalidade terapêutica. Relato do caso: Trata-se de um homem, 65 anos, com diagnóstico de carcinoma de células escamosas em assoalho bucal esquerdo, tratado com cirurgia e radioterapia adjuvante. Na avaliação odontológica inicial, não foram observadas alterações clínicas ou radiográficas. Duas semanas após o término da radioterapia, o paciente relatou ter acordado com dor intensa em mandíbula, sem relato de trauma ou queda. A radiografia panorâmica evidenciou fratura no corpo mandibular esquerdo, sugerindo fratura idiopática durante o sono. Após dez dias, houve exposição óssea intraoral do coto distal e preconizou-se tratamento conservador com analgesia, osteotomia superficial sob anestesia local e antibioticoterapia profilática. O paciente evoluiu com secreção purulenta, fístula extraoral e eliminação de sequestro ósseo, após cinco meses, confirmando o diagnóstico de osteorradionecrose. Diante desse quadro, após 11 meses do diagnóstico da fratura, optou-se pela intervenção cirúrgica de mandibulectomia redutora de coto distal. Depois de sete meses de acompanhamento pós-cirúrgico, o paciente encontra-se sem evidências clínicas e radiográficas de osteorradionecrose. Conclusão: O tratamento da osteorradionecrose é considerado desafiador para os dentistas que lidam com essa sequela da radioterapia. Portanto, destaca-se a importância da capacitação do dentista para atuar em todas as etapas do tratamento oncológico


Introduction: Osteoradionecrosis of the jaws affects 1% to 6% of patients undergoing radiotherapy and is considered the most severe oral complication resulting from this therapeutic modality. Case report: This is a 65-year-old man diagnosed with squamous cell carcinoma in the left oral floor, treated with surgery and adjuvant radiotherapy. In the dental evaluation, no clinical or radiographic changes were observed. Two weeks after the radiotherapy, the patient reported severe pain in the mandible, with no report of trauma or fall. The panoramic radiograph showed a fracture in the left mandibular body, suggesting an idiopathic fracture while asleep. After ten days, there was intraoral bone exposure of the distal portion of the mandible and conservative treatment with analgesia, superficial osteotomy under local anesthesia and prophylactic antibiotic therapy was performed. The patient evolved with purulent secretion, extraoral fistula and elimination of bone sequestration, after five months, confirming the diagnosis of osteoradionecrosis. Thereby, 11 months after the fracture diagnosis, the patient underwent a surgical intervention with partial mandibulectomy of the distal portion. After seven months of post-surgical follow-up, the patient presents neither clinical or radiographic evidence of osteoradionecrosis. Conclusion: The treatment of osteoradionecrosis is considered challenging for dentists who deal with this side effect of radiotherapy. Therefore, the importance of training the dentist to work in all stages of cancer treatment is highlighted.


Introducción: La osteorradionecrosis afecta del 1% al 6% de los pacientes sometidos a radioterapia y se considera la complicación oral más grave resultante de esta modalidad terapéutica. Relato del caso: Hombre, 65 años, diagnosticado con carcinoma de células escamosas en el suelo de boca izquierdo, tratado con cirugía y radioterapia adyuvante. La evaluación odontológica no presentó alteraciones clínicas o radiográficas. Dos semanas después de concluir la radioterapia, él informó haber despertado con un fuerte dolor en la mandíbula, sin historia de trauma o caída. La radiografía panorámica mostró una fractura en el cuerpo mandibular izquierdo, lo que sugirió una fractura idiopática durante el sueño. Diez días después, hubo exposición ósea intraoral del muñón distal y fue empleado tratamiento conservador con analgesia, osteotomía superficial bajo anestesia local y antibiótico profiláctico. El paciente evolucionó con secreción purulenta, fístula extraoral y eliminación de secuestro óseo, pasados cinco meses, confirmando el diagnóstico de osteorradionecrosis. Así, pasados 11 meses del diagnóstico de fractura, fue indicada intervención quirúrgica de mandibulectomía reductora del muñón distal. Después de siete meses de la cirugía, no hay evidencias clínicas o radiográficas de osteorradionecrosis. Conclusión: El tratamiento de la osteorradionecrosis se considera un desafío para los dentistas que se ocupan de esta secuela de la radioterapia. Por lo tanto, se destaca la importancia de capacitar al dentista, para que trabaje en todas las etapas del tratamiento oncológico

Background: Squamous cell carcinoma (SCC) of head and neck is highly prevalent in South-asian countries, owing to high consumption of areca nut/gutka and chewing tobacco. p27kip1 is a tumor suppressor gene, thought to be downregulated in oral squamous cell carcinoma. Therefore, in the present study we used immunohistochemical analysis to investigate an association between low p27kip1 expression in SCC of the head and neck and adverse outcomes/risk factors. Methods: Total 105 cases of SCC of head and neck excision specimens were selected from records of pathology department archives that underwent surgeries at Liaquat National hospital, Karachi from January 2008 till December 2013. Clinical and pathologic characteristics of patients were evaluated and p27kip1 immunohistochemistry was applied on tumor blocks. Results: In our study, low expression of p27kip1 in SCC of head and neck was seen in 39(37.1%) cases while 66(62. 9%) of the cases showed high expression for p27kip1. Significant association of p27kip1 expression with pan/gutka usage (p = 0.004), and recurrence (p = 0.001) was noted; however, no significant association of p27kip1 expression with other clinicopathologic features was seen. Multivariate binary logistic regression showed cases with history of pan/gutka usage were more likely to show low p27kip1 expression. Similarly, we also found that recurrence was more likely to develop in patients with low expression of p27kip1 in comparison to cases showing high p27kip1 expression. Conclusion: Loss of p27kip1 expression is a significant event involved in the pathogenesis of SCC head and neck especially that of oral cavity. Significant association of gutka/areca nut with low p27kip1 expression in our study suggests that loss p27kip1 expression is a major event involved in areca nut induced SCC of head and neck in this part of the world; however, more large scale molecular based studies are required to validate this observation. Moreover, significant association of low p27kip1 expression with tumor recurrence suggests its importance as a prognostic biomarker in SCC of head and neck (AU)

Abstract Introduction Critical weight loss is defined as an unintentional weight loss of ≥ 5% at 1 month or ≥ 10% at 6 months from the start of treatment. Critical weight loss leads to deterioration of the immune function and reduced tolerance to treatment (surgery ± radiochemotherapy) as well as increased complication rates. Objective Critical weight loss, defined as a weight loss of ≥ 5% after 1 month or ≥ 10% after 6 months from the start of treatment, is not uncommon in head and neck cancer patients. We aimed to assess the factors associated with critical weight loss during the treatment of oral cavity squamous cell carcinoma patients. Methods A retrospective cohort study was performed at the Aga Khan University Hospital, in Karachi, Pakistan, on 125 patients. Patients receiving adjuvant therapy were considered exposed, and the outcome was critical weight loss. Results The mean age of presentation was 46.9 ± 12.8 years in patients undergoing surgery and adjuvant therapy, with 119 (79.3%) of them being male and 31 (20.7%) female. One hundred and twelve patients (81.3%) developed critical weight loss at 6 months from the start of treatment, and the only significant variable associated with critical weight loss was the stage of the disease (p= 0.03). Conclusion A large proportion of patients with oral cancer developed critical weight loss requiring a need for intervention. The overall stage of the disease is a significant predictor of critical weight loss in patients undergoing treatment.

ABSTRACT

O carcinoma epidermoide corresponde a 90% das lesões malignas da cavidade oral e orofaringe. Entre os fatores de risco para o câncer de boca pode-se destacar o tabagismo e o etilismo. Além disso, vírus oncogênicos estão sendo relacionados ao desenvolvimento do câncer ral e de orofaringe. Entre estes vírus, destaca-se o papilomavírus humano. Avaliar o efeito carcinogênico do papilomavírus humano em cavidade oral e orofaringe. Uma busca sistemática de artigos científicas publicados entre janeiro de 2000 a maio de 2016 foi executada nas bases de dados PubMed/Medline, Scopus e Science Direct através dos descritores: Câncer Oral e de Orofaringe, Carcinoma Epidermoide, papiloma vírus Humano. Após análise completa das publicações, seis artigos preencheram todos os critérios de inclusão. Em nossa revisão sistemática, o subtipo 16 foi o mais frequentemente localizado em cavidade oral nos pacientes com carcinoma epidermoide em todos os estudos, sendo de acordo com a literatura científico um subtipo de alto risco para o desenvolvimento neoplásico. O papilomavírus humano pode apresentar-se como um fator etiológico para o desenvolvimento do câncer de orofaringe, assim como para um subgrupo de carcinoma epidermoide, essencialmente em casos de infecção pelo subtipo de risco papilomavírus humano 16. The Squamous cell carcinoma accounts for 90% of malignant lesions of the oral cavity and oropharynx. Among the risk factors for oral cancer can highlight smoking and alcohol consumption. In addition, oncogenic viruses are related to the development of oral cancer and oropharyngeal. To evaluate the carcinogenic effect of human papillomavirus in oral cavity and oropharynx. A systematic search of scientific articles published from January 2000 to May 2016 was performed in the databases PubMed / Medline, Scopus and Science Direct through the descriptors: Oral and oropharynx cancer, squamous cell carcinoma, Human Papilloma Virus. After complete analysis of publications, six articles met all inclusion criteria. In our systematic review, subtype 16 was most often located in the oral cavity in patients with squamous cell carcinoma in all studies, and according to the scientific literature a high risk subtype for neoplastic development. Human papilloma vírus can be presented as an etiological factor for the development of oropharyngeal cancer as well as squamous cell carcinoma subgroup, mainly in cases of infection risk subtype of human papilloma vírus 16. ABSTRACT

Se revisó en la literatura científica la importancia del odontólogo dentro del plantel multidisciplinario de oncología en artículos científicos extraídos de bases de datos tales como: PubMed, SciELO, CrossRef, Revistas USC, Revista Científica Odontológica, Elsevier, Researchgate, Google Académico, Wiley Online Library y Springer. En base a 32 artículos seleccionados se concluye que el odontólogo como parte del plantel multidisciplinario de oncología tiene un papel sumamente importante, ya que su integración permite contrarrestar las distintas complicaciones orales que se presentan a consecuencia de las diferentes terapias antineoplásicas a las cuales son sometidos los pacientes con cáncer. Estas complicaciones orales deben ser indefectiblemente tratadas, pues, su presencia en la cavidad oral constituye un riesgo aún mayor en cuanto a infección sistémica se refiere. He aquí la importancia del odontólogo dentro del plantel multidisciplinario de oncología evitando y previniendo la propagación de focos infecciosos y mejorando así la calidad de vida de los pacientes oncológicos.


The importance of the dentist within the oncology multidisciplinary staff in scientific articles extracted from databases such as PubMed, SciELO, CrossRef, USC Journals, Dental Scientific Journal, Elsevier, Researchgate, Google Scholar, Wiley Online Library and Springer was reviewed. Based on 32 selected articles, it is concluded that the dentist as part of the oncology multidisciplinary team plays an extremely important role, since its integration allows counteracting the different oral complications that arise as a consequence of the different antineoplastic therapies to which the cancer patients are subjected. These oral complications must be treated unfailingly, since their presence in the oral cavity constitutes an even greater risk as far as systemic infection is concerned. Here is the importance of the dentist within the oncology multidisciplinary staff, avoiding and preventing the spread of infectious foci and thus improving the quality of life of cancer patients.

SUMMARY Oral melanoacanthoma is a mucocutaneous, pigmented, rare, benign, and probably reactive lesion. This paper reports for the first time in the literature a case of multifocal oral melanoacanthoma in a patient diagnosed with Addison's disease and concomitant Graves' disease with hyperthyroidism. The patient presented with oral pigmented lesions, which were hypothesized to be mucosal pigmentation associated with Addison's disease. Due to their unusual clinical pattern, these oral lesions were biopsied and diagnosed as oral melanoacanthoma on histopathology and immunohistochemistry for HMB-45. At the moment of this report, the patient was being treated for her systemic conditions, but the lesions had not regressed. Reactive hyperpigmentation of the skin and mucous membranes may be found in Addison's disease and hyperthyroidism. This case reinforces the hypothesis of a reactive nature for oral melanoacanthoma and highlights the need for investigation of endocrine disorders in patients with multifocal oral melanoacanthoma.

Introducción: el conocimiento por parte de la población del cáncer bucal es de vital importancia para actuar sobre los factores de riesgo a padecer cáncer bucal y disminuir su incidencia. Objetivo: evaluar el nivel de conocimiento de la población objeto de estudio sobre los factores de riesgo del cáncer bucal, los factores protectores para prevenir el cáncer bucal y los signos bucales de alarma relacionados con el cáncer bucal. Materiales y Métodos: se realizó un estudio experimental de intervención educativa para aumentar el nivel de conocimiento y la percepción del riesgo ante el cáncer bucal, en el territorio atendido por el Hospital General Docente “Julio Aristegui Villamil” de Cárdenas. Resultados: se diagnosticó bajo nivel de conocimiento en el 78,5 % de la población estudiada sobre el cáncer bucal, se diseñó y ejecutó una estrategia de intervención educativa para aumentar la percepción del riesgo ante el cáncer bucal en la población según las necesidades de aprendizaje. El nivel de conocimiento luego de la intervención educativa resultó satisfactorio en el 94,2 % de la población. Conclusiones: el nivel de conocimiento sobre cáncer bucal inicial resultó ser insatisfactorio en la mayoría, mientras que luego de aplicar la intervención educativa fue satisfactorio en casi su totalidad. La intervención educativa resultó ser un método idóneo de la educación para la salud respecto al cáncer bucal en la población estudiada (AU).


Introduction: knowledge of oral cancer in all the population is very important, that permit to act over factors of risk to suffer Oral cancer and to diminish the morbidity in this illness. Objective: to describe the level knowledge about factors of risk of Oral cancer in the population studied, the protective factors to prevent the Oral cancer and the alarm signs related with the Oral cancer; as well as to evaluate the level of knowledge before and after the application of the educational strategy to the population. Materials and methods: was carried out an experimental study of educational intervention to increase the perception of risk about Oral cancer, in the territory assisted by the General Hospital "Julio Aristegui Villamil" of Cárdenas. The level of knowledge about Oral cancer was studied in 450 patients. Results: low level of knowledge was diagnosed in population´s studied (78,5 %). Then was designed and executed a Strategy of Educational Intervention to increase the perception of the risk about Oral cancer in the population according to the learning necessities. The level of knowledge after the educational intervention was Satisfactory in the 94,2 % of the population. Conclusions: at first most of population have a not satisfactory level of knowledge about Oral cancer, but it turned out to be Satisfactory in almost totally, after applying the educational intervention. This educational intervention turned out to be a suitable method for the health´s education regarding Oral cancer in the studied population (AU).

Introducción: La región bucal es una de las diez localizaciones más frecuentes de incidencia del cáncer en Cuba y en el mundo, su estudio en nuestro medio es de vital importancia para mejorar su prevención y curación. Objetivo: identificar distribución de morbilidad por año del cáncer bucal en el territorio de Cárdenas y su tasa por 100 000 habitantes, características sociodemográficas de los pacientes diagnosticados con cáncer bucal en la región y periodo objetos de estudio y determinar el comportamiento del cáncer bucal según localización anatómica y presencia de factores de riesgo. Materiales y Métodos: se realizó un estudio descriptivo retrospectivo de 15 años en pacientes con diagnóstico histopatológico de cáncer bucal atendidos en el Hospital General Docente ¨Julio M Aristegui Villamil¨ de Cárdenas, en el periodo comprendido entre enero de 2000 y diciembre de 2014. Resultados: se pudo constatar un incremento de pacientes afectados en el referido territorio al elevarse la tasa de 4,5 X 100 000 habitantes en el 2000 a 15,4 en el 2013 y 12,5 en el 2014. Hubo mayor incidencia en el grupo de edad de 51-60 años (24,6 %), el sexo masculino (78,5 %), el color de piel blanca (94,1 %), el labio inferior (44,5 %). Los factores de riesgo con mayores valores fueron el tabaquismo (94,6 %) y alcoholismo (64,8 %). Conclusiones: El cáncer bucal tiene una tendencia al incremento en el municipio estudiado, predominando en individuos masculinos, de piel blanca y con adicciones como tabaquismo y alcoholismo. Es necesario realizar actividades de promoción de salud y prevención del cáncer bucal por el incremento de esta enfermedad (AU).


Introduction: oral region is one of the ten more frequent localizations of the cancer in our country and in the world, so to study oral cancer in our context is very important to improve prevention and cure. Objective: describing the distribution of the morbidity for year of the Oral cancer in Cárdenas and their rate for 100 000 inhabitants, as well as the demographic characteristics of the patients diagnosed with Oral cancer in the anatomical region and period studied and factors of risk in those patients. Materials and methods: was carried out a descriptive and retrospective study of 15 year in patient with histopathologic diagnosis of Oral cancer assisted in the General Hospital ¨Julio M Aristegui Villamil¨ of Cárdenas, since January, 2000 to December, 2014. Results: was verified an increase of patients affected by Oral cancer in the referred territory and was raising the rate of since 4,5 X 100 000 inhabitants in the 2000 to 15,4 in the 2013 and 12,5 in the 2014. There was bigger incidence in the group of 51-60 year-old age (24,6 %), the masculine sex (78,5 %), the white skin color (94,1 %) and the inferior lip area (44,5 %). The higher factors of risk were Tabaco addiction (94,6 %) and alcoholism (64,8 %). Conclusions: it becomes necessary to carry out activities of promotion of health and prevention of Oral cancer because the growth of this illness in our context (AU).

ABSTRACT

Los odontomas están compuestos de esmalte, dentina, cemento y tejido pulpar. Histológicamente se clasifican en 2 grupos: compuestos y complejos, mientras que clínicamente se clasifican en 3 tipos: odontoma central (intraóseo), odontoma periférico (extraóseo o de los tejidos blandos) y odontoma erupcionado. Epidemiológicamente son reportados como los tumores odontogénicos más frecuentes, sin embargo la erupción de este tipo de lesión es poco común, produciéndose en el 1,6% de los casos. Por lo general son asintomáticos, siendo comúnmente un hallazgo radiográfico, y en algunos casos pueden estar asociados con alteraciones de la erupción dentaria. Este estudio corresponde a un caso clínico de un odontoma complejo de gran tamaño que erupciona en la cavidad oral asociado a un molar retenido.


Odontomas consist of enamel, dentin, cementum, and pulp tissue. Histologically, they are classified into 2 groups: compounds and complexes. They are also clinically classified into 3 types: Central odontoma (intraosseous), peripheral odontoma (extraosseous or soft tissue odontoma) and erupted odontoma. Epidemiologically they are reported as the most frequent odontogenic tumor, however, the eruption of this type of lesion is rare, and odontoma occurs only in 1.6% of cases.

ABSTRACT

Tumor related tissue eosinophilia (TATE) is a known phenomenon but its role in prognostics and correlation with size of the primary tumor is still controversial. Using a stain, like Carbol chromotrope, that targets eosinophils exclusively and vividly, offers an advantage over haematoxylin and eosin, which was used in most of the studies. Forty-nine cases of oral squamous cell carcinoma, where the TNM staging has been recorded in their history, was taken and stained with Lendrum's carbol chromotrope. Significant difference in the eosinophil count with varying size of the tumor and a parallel increase in the number noted, with increase in size. There is a corresponding increase in the number of eosinophils infiltrating the tumor with increase in size of oral squamous cell carcinoma.


La eosinofilia tisular asociada a tumores (TATE) es un fenómeno conocido, sin embargo su pronóstico y correlación con el tamaño del tumor primario aún es tema de controversia. El uso de cromotropo como tinción dirigida exclusivamente a los eosinófilos, ofrece una ventaja sobre la hematoxilina-eosina, que ha sido utilizada en la mayoría de los estudios. Se estudiaron células escamosas en 49 casos de carcinoma oral, con registro del estadio TNM. Las células fueron teñidas con carbol cromotropo de Lendrum. Se observó una diferencia significativa en el recuento de eosinófilos con el tamaño del tumor y un aumento paralelo en número, con el aumento de tamaño. Hay un aumento correspondiente en el número de eosinófilos que infiltran el tumor con aumento en el tamaño de carcinoma de células escamosas orales.

ABSTRACT

Os genes homeobox atuam como reguladores da morfogênese e diferenciação celular embrionária, portanto, é evidente a possibilidade de sua expressão anormal estar presente na progressão de tumores. Estudos preliminares em nosso laboratório verificaram a participação de alguns genes homeobox em carcinoma epidermóide de boca (CEB). Este trabalho teve como objetivo avaliar a amplificação, expressão e o perfil de metilação dos genes HOXA5, HOXA7, HOXA9, HOXB5, HOXB13, HOXC12 em linhagem celular derivadas de CEB e tecidos frescos tumoral e não-tumoral. Além disso, verificar o efeito da desmetilação na expressão gênica de linhagens celulares que apresentaram genes 100% metilados e a participação das enzimas responsáveis pela metilação do DNA, bem como a expressão das DNAmetiltransferases (DNMT) nos tumores. A análise de amplificação do DNA e expressão de mRNA foi realizada por qRT-PCR. O perfil de metilação foi avaliada pelo sistema PCR Array e a análise proteica das DNMT1, DNMT3a, DNMT3b e HOXA9 foi verificada por meio de reações imunohistoquímicas. As linhagens celulares SCC4 e SCC9 foram utilizadas para análise de desmetilação com 5-aza-2'-deoxicitidina e a linhagem SCC4 para avaliar os efeitos do aumento de expressão do HOXA9, na proliferação celular por imunocitoquimica para Ki67, migração celular por transwell e apoptose pela avaliação de células positivas no ensaio de TUNEL. Na comparação entre os grupos, o gene HOXA5 apresentou-se amplificado na margem em relação ao tumor; o HOXA9 apresentou nível de metilação aumentada no tumor; o HOXB5 com amplificação maior na margem, com nível de expressão do mRNA aumentada nos tumores, e nível de metilação do tumor maior em relação a margem, sendo correlacionada com menor sobrevida; HOXB13 se apresentou amplificado no tumor em relação a margem, e com nível de metilação maior nos tumores e,


HOXC12 com níveis de metilação maior nos tumores em relação a margem. É interessante, que os mesmos genes que tiveram níveis de metilação aumentados nos tumores em relação a margem, também estavam 100% metilados nas linhagens celulares SCC4 e SCC9 e tiveram sua expressão restaurada após o tratamento com 5-aza-2´-deoxicitina. Na avaliação do nível de expressão das DNMTs nos tumores, a DNMT3b apresentou-se com níveis aumentados em relação a DNMT1 e DNMT3a, e quando avaliado em nível proteico, a DNMT3a pode ser correlacionada com melhor sobrevida. O aumento da expressão do gene HOXA9 mostrou diminuição da migração celular, porém não alterou a proliferação e apoptose celular. A expressão proteica não apresentou correlação com parâmetros clínicos. Em conclusão, os resultados mostram que os genes homeobox estudados estão pouco metilados nas linhagens celulares e em tecidos de CEB. A amplificação desses genes não é um evento frequente. O gene HOXA9 é pouco expresso no tumor, e o aumento da sua expressão em linhagens celulares diminui a migração celular.


Homeobox genes are important as morphogenetic and embrionary cellular differentiation regulators, therefore there is basis for their abnormal expression in tumor progression. Preliminary studies in our laboratory have shown that homeobox genes are dysregulated in oral squamous cell carcinoma (OSCC). This study evaluates the genomic amplification, mRNA expression and methylation status of HOXA5, HOXA7, HOXA9, HOXB5, HOXB13, HOXC12 in squamous cell carcinoma derived cell lines, and fresh tumor tissue. Also, analyzes the demethylation effect in gene expression of cell lines with genes showing 100% methylation, and the participation of enzymes responsible for DNA methylation, DNAmetiltransferases (DNMT), in gene and protein expression in tumors. DNA amplification and mRNA expression was analyzed by qRT-PCR. The methylation profile was evaluated by PCR Array System and DNMT1, DNMT3a, DNMT3b and HOXA9 protein expression was verified by immunohistochemistry. SCC4 and SCC9 cell lines were submitted to 5-aza-2'-deoxycytidine for demethylation analysis. SCC4 cell lineage was analyzed by immunocytochemistry for Ki67, cell migration by transwell and apoptosis by TUNEL test after increased expression of HOXA9. HOXA5 gene was amplified in the adjacent margin when compared with the tumor; HOXA9 showed increased level of methylation in tumor; HOXB5 showed amplification in the margin, increased mRNA expression in tumors, increased methylation level and was


correlated with decreased survival; HOXB13 was amplified in tumor samples when compared to the margins and also higher methylation level in tumors; and HOXC12 also showed increased methylation levels in tumors when compared to margin. Interestingly, the same genes with increased methylation levels in tumors, were also 100% methylated in cell lines SCC4 and SCC9. The expression of these gens was restored after treatment with 5-aza-2'-deoxycytidine. DNMT3b presented higher levels of protein expression relative to DNMT1 and DNMT3a. DNMT3a protein expression was correlated with improved survival. SCC4 cells overexpressing HOXA9 gene showed decreased cell migration, with no effect on cell proliferation and apoptosis. HOXA9 protein expression was not correlated with clinical parameters. In conclusion, the results shows that homeobox genes are methylated in some OSCC cell lines and tissues. Amplification of these genes is not a frequent event. HOXA9 gene has low expression in OSCC, and when overexpressed in cell lines decreases cell migration.

Quatro décadas de pesquisas mostraram que muitos mecanismos inflamatórios estão intrinsecamente ligados ao desenvolvimento e manutenção do câncer, e ainda, que as citocinas inflamatórias exercem papel primordial nessa relação. Os anti-inflamatórios não esteroides (AINEs) podem reduzir o desenvolvimento neoplásico por afetar a produção de citocinas inflamatórias pelas células neoplásicas. No entanto, até o momento não foi bem definido se o tratamento com AINEs é capaz de modular a expressão de citocinas inflamatórias por células do carcinoma epidermoide oral (CEO). O objetivo deste trabalho foi avaliar a expressão de citocinas inflamatórias em linhagens celulares de CEO após tratamento com ácido acetilsalicílico (AAS) e celecoxibe (CLX). Foi realizado screening da expressão de 84 citocinas e quimiocinas, através de PCR array, das linhagens SCC4, 9 e 25 tratadas com doses de AAS e CLX próximas às concentrações plasmáticas dos fármacos em humanos. Os resultados mostraram que AAS e CLX modularam a expressão de citocinas e que as linhagens responderam de maneira diferente aos tratamentos. Observou-se aumento de expressão de citocinas pró-inflamatórias como a IL-1?, IL-8 e TNF na SCC9 e 25, assim como diminuição de expressão de ACKR4 e CXCL10 na SCC4 e 9.


Four decades of research have shown that many inflammatory mechanisms are intrinsically linked to the development and maintenance of cancer and that inflammatory cytokines play pivotal role in this association. Non-steroidal anti-inflammatory drugs (NSAIDs) can reduce neoplastic growth on affecting the production of inflammatory cytokines by the neoplastic cells. So far, it is not well established if the treatment with NSAIDs can modulate the expression of inflammatory cytokines by OSCC cells. The objective of this study was to evaluate the expression of inflammatory cytokines by OSCC cell lines after treatment with acetylsalicylic acid (ASA) and celecoxib (CLX). Eighty-four cytokines and chemokines mRNA expression were screened by PCR array on SCC4, 9 and 25 cell lines treated with ASA and CLX at plasma concentrations in humans. The results showed that ASA and CLX modulate the expression of cytokines with all cell lines responding differently to the treatments. Increased expression of proinflammatory cytokines such as IL-1?, IL-8 and TNF in SCC9 and 25, and reduced expression of ACKR4 and CXCL10 in SCC4 and 9, was observed. Thus it follows that the treatments of lines SCC4, SCC9 and SCC25 with ASA and CLX at the next plasma concentrations in humans are able to modulate the gene expression of inflammatory cytokines.

ABSTRACT

Os tumores odontogênicos formam um grupo complexo de lesões, de comportamento clínico variado e encontrado exclusivamente na região bucomaxilofacial. Originam-se a partir das células que normalmente teriam a função de formar o dente, processo esse denominado odontogênese, que tem seu início entre a sexta e a sétima semanas de vida intrauterina. O objetivo deste trabalho foi realizar um estudo clinicopatológico de tumores odontogênicos diagnosticados no Serviço de Patologia Cirúrgica da Faculdade de Oodontologia da Universidade de São Paulo de 1957 a 2012. Foram compilados das fichas de requisição de exames e/ou prontuários dados referentes ao sexo, idade no momento do diagnóstico, cor da pele (branca, negra, ou outras), sintomatologia, tamanho da lesão, duração, localização anatômica, aspectos radiográficos, diagnóstico clínico e histopatológico. Os casos diagnosticados como tumores odontogênicos foram revisados, por um examinador previamente calibrado, com o objetivo de reclassificação das lesões conforme proposição atual da Organização Mundial da Saúde em 2005. Todos os dados obtidos foram inseridos e tabulados no Microsoft Office Excel 2013 e analisados no software BioEstat 5.0. Medidas de frequência central e de dispersão foram obtidas e os resultados apresentados em forma de tabelas e gráficos. Na análise estatística, foi empregado o teste Z de duas proporções com nível de significância de 5%. No período compreendido de 01 de janeiro de 1957 a 31 de dezembro de 2012, foram diagnosticados 2.114 casos de tumores odontogênicos.


Do total de casos diagnosticados, 75 casos foram retirados da análise, pois representavam a mesma lesão em diferentes momentos. Portanto, 2.039 casos de tumores odontogênicos foram incluídos e, após revisão dos casos e reclassificação diagnóstica dos tumores odontogênicos, os 2.039 casos foram agrupados em 16 tipos histológicos diferentes, entre os quais 2.029 (99,51%) eram benignos e 10 (0,49%) malignos. De uma maneira geral, os tumores odontogênicos afetam pacientes na segunda e terceira décadas de vida, sem predileção por sexo, apresentam-se menores do que 1cm de diâmetro e são diagnosticados, em média, 2 anos após seu surgimento. A maioria das lesões acometem a região posterior da mandíbula, com exceção do odontoma e tumor odontogênico adenomatóide, e apresentam aspecto radiográfico radiolúcido inespecífico, o que dificulta um diagnóstico preciso sem a avaliação anatomopatológica.


Odontogenic tumors are a complex group of lesions, varied clinical behavior and found exclusively in the oral and maxillofacial region. Originate from cells that would normally have the function of forming the tooth, a process called odontogenesis, which begins between the sixth and seventh weeks of intrauterine life. The aim of this study was a clinicopathological study of odontogenic tumors diagnosed in Surgical Pathology Service of Oodontologia of the University of São Paulo from 1957 to 2012 were compiled from surveys of request forms and / or records data on gender, age at diagnosis, skin color (white, black, or other), symptoms, lesion size, duration, anatomic location, radiographic findings, clinical and histopathological diagnosis. The cases diagnosed as odontogenic tumors were reviewed by a calibrated examiner, in order to reclassify injuries as current proposition of the World Health Organization in 2005. All data were entered and tabulated in Microsoft Office Excel 2013 and analyzed in software BioEstat 5.0. Central frequency and dispersion measurements were obtained and the results presented in tables and graphs. In the statistical analysis, we used the Z test for similar proportions with 5% significance level. In the period of January 1, 1957 to December 31, 2012 were diagnosed 2,114 cases of odontogenic tumors. Of all diagnosed cases, 75 cases


were removed from the analysis because they represented the same injury at different times. Therefore, 2,039 odontogenic tumors were included and, after review of the cases and diagnostic reclassification of odontogenic tumors, 2,039 cases were grouped into 16 different histological types, including 2 029 (99.51%) were benign and 10 (0, 49%) malignant. In general, the odontogenic tumors affect patients in the second and third decades of life, with no gender preference, are presented less than 1cm in diameter and are diagnosed on average two years after its inception. Most injuries involve the posterior mandible, except for the odontoma and adenomatoid odontogenic tumor, and feature radiolucent radiological findings nonspecific, which hinders accurate diagnosis without pathological assessment.

HOXA5 gene was amplified in the adjacent margin when compared with the tumor; HOXA9 showed increased level of methylation in tumor; HOXB5 showed amplification in the margin, increased mRNA expression in tumors, increased methylation level and wascorrelated with decreased survival; HOXB13 was amplified in tumor samples when compared to the margins and also higher methylation level in tumors; and HOXC12 also showed increased methylation levels in tumors when compared to margin. Interestingly, the same genes with increased methylation levels in tumors, were also 100% methylated in cell lines SCC4 and SCC9. The expression of these gens was restored after treatment with 5-aza-2'-deoxycytidine. DNMT3b presented higher levels of protein expression relative to DNMT1 and DNMT3a. DNMT3a protein expression was correlated with improved survival. SCC4 cells overexpressing HOXA9 gene showed decreased cell migration, with no effect on cell proliferation and apoptosis. HOXA9 protein expression was not correlated with clinical parameters. In conclusion, the results shows that homeobox genes are methylated in some OSCC cell lines and tissues. Amplification of these genes is not a frequent event. HOXA9 gene has low expression in OSCC, and when overexpressed in cell lines decreases cell migration.

Quatro décadas de pesquisas mostraram que muitos mecanismos inflamatórios estão intrinsecamente ligados ao desenvolvimento e manutenção do câncer, e ainda, que as citocinas inflamatórias exercem papel primordial nessa relação. Os anti-inflamatórios não esteroides (AINEs) podem reduzir o desenvolvimento neoplásico por afetar a produção de citocinas inflamatórias pelas células neoplásicas. No entanto, até o momento não foi bem definido se o tratamento com AINEs é capaz de modular a expressão de citocinas inflamatórias por células do carcinoma epidermoide oral (CEO). O objetivo deste trabalho foi avaliar a expressão de citocinas inflamatórias em linhagens celulares de CEO após tratamento com ácido acetilsalicílico (AAS) e celecoxibe (CLX). Foi realizado screening da expressão de 84 citocinas e quimiocinas, através de PCR array, das linhagens SCC4, 9 e 25 tratadas com doses de AAS e CLX próximas às concentrações plasmáticas dos fármacos em humanos. Os resultados mostraram que AAS e CLX modularam a expressão de citocinas e que as linhagens responderam de maneira diferente aos tratamentos. Observou-se aumento de expressão de citocinas pró-inflamatórias como a IL-1?, IL-8 e TNF na SCC9 e 25, assim como diminuição de expressão de ACKR4 e CXCL10 na SCC4 e 9.


Four decades of research have shown that many inflammatory mechanisms are intrinsically linked to the development and maintenance of cancer and that inflammatory cytokines play pivotal role in this association. Non-steroidal anti-inflammatory drugs (NSAIDs) can reduce neoplastic growth on affecting the production of inflammatory cytokines by the neoplastic cells. So far, it is not well established if the treatment with NSAIDs can modulate the expression of inflammatory cytokines by OSCC cells. The objective of this study was to evaluate the expression of inflammatory cytokines by OSCC cell lines after treatment with acetylsalicylic acid (ASA) and celecoxib (CLX). Eighty-four cytokines and chemokines mRNA expression were screened by PCR array on SCC4, 9 and 25 cell lines treated with ASA and CLX at plasma concentrations in humans. The results showed that ASA and CLX modulate the expression of cytokines with all cell lines responding differently to the treatments. Increased expression of proinflammatory cytokines such as IL-1?, IL-8 and TNF in SCC9 and 25, and reduced expression of ACKR4 and CXCL10 in SCC4 and 9, was observed. Thus it follows that the treatments of lines SCC4, SCC9 and SCC25 with ASA and CLX at the next plasma concentrations in humans are able to modulate the gene expression of inflammatory cytokines.

ABSTRACT

O carcinoma epidermóide de boca (CEB) é uma neoplasia agressiva, causadora de alta morbidade e mortalidade. Uma de suas características clínicas mais importantes é a disseminação tumoral levando a uma doença metastática nos linfonodos regionais. Entretanto, limitações relacionadas às tecnologias diagnósticas atualmente disponíveis fazem com que a detecção da metástase linfonodal ainda seja um grande desafio clínico. Assim, melhorias na capacidade de predição de potencial metastático e/ou na detecção precoce e precisa da doença linfonodal possibilitariam determinar quais pacientes, de fato, se beneficiariam da abordagem cirúrgica cervical. Ao mesmo tempo, preservaria os portadores de tumores menos agressivos (não-metastáticos), reduzindo a morbidade e levando à maior sobrevida. Neste estudo investigamos padrões de expressão gênica tumoral determinados a partir do sequenciamento em larga escala do transcriptoma, comparando, por RNA-Seq, 10 CEB pequenos, mas já metastáticos (T1N+), com 10 CEB grandes, mas não-metastáticos (T3/T4N0). Análises de bioinformática sugeriram 150 genes diferencialmente expressos (DE) com potencial de separar os dois grupos de estudo. O valor preditivo destes genes foi desafiado in silico com dados do The Cancer Genome Atlas (TCGA), seguido de validação técnica nas mesmas amostras sequenciadas e, finalmente, validação biológica a partir de amostras independentes de CEB. Nossos resultados indicaram o NR3C2 como um potencial biomarcador de metástase nodal e sugerem que a regulação de canais iônicos pode ter um papel importante no processo metastático do CEB. Embora mais estudos sejam necessários, nossos resultados indicam que no futuro marcadores transcricionais poderão ser usados no diagnóstico e predição da ocorrência de doença metastática linfonodal em pacientes com CEB, promovendo melhorias na abordagem terapêutica.


Oral squamous cell carcinoma (OSCC) is an aggressive disease related to high morbidity and mortality. Lymph node metastasis is one of the main prognostic factors for OSCC patients, but no reliable diagnostic methods are available to accurately diagnose metastatic disease. Improvements in diagnosis of metastasis would enhance the indication of neck surgery for lymph nodes resection, sparing patients with locally restricted disease. In this study, we aimed to investigate if differences in the pattern of gene expression would help to diagnose metastatic OSCC. For this, we sequenced the transcriptome of 10 small (T1) metastatic and 10 large (T3-T4) nonmetastatic OSCC aiming to determine differentially expressed transcripts (DET). Our results showed 150 DET with potential to separate metastatic and non-metastatic OSCC cases. This set of 150 genes were challenged through an in silico approach using data from The Cancer Genome Atlas (TCGA). Validated genes were then confirmed by RT-qPCR using the same set of sequenced samples followed by another confirmation in an independent set of OSCC cases. The transcript NR3C2 was found as a putative biomarker for lymph node metastasis. Pathway analysis indicated that transcriptional alterations related to ion channels pathways may be involved in the biology of metastasis in OSCC. Although our results are not definitive, it indicates that transcriptional markers would be useful to diagnose or predict lymph node metastatic disease in OSCC patients, potentially improving therapeutic approaches.

A Organização Mundial de Saúde (OMS) definiu a qualidade de vida como a percepção do indivíduo sobre si, no contexto de valores em que vive e em relação a: metas, expectativas, parâmetros e relações sociais. O objetivo do estudo foi realizar a equivalência dos instrumentos de qualidade de vida (University of Washington - Quality of life (UW-QOL), Brief Core Set Questionnaire - Head and Neck - BCSQ H&N), em pacientes com câncer de boca, diagnosticados na Liga de Neoplasia Bucal (LINB) e o Oral Health Impact Profile (OHIP-14) para pacientes que buscaram por atendimento de urgência, com a Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF). Foram entrevistados 50 pacientes atendidos pela Liga de Neoplasias da FOUSP e 100 pacientes do Setor de Urgências da Clínica Odontológica da FOUSP. Os escores de cada instrumento foram pontuados e, em seguida, verificou-se a frequência de ocorrência dos domínios. As categorias mais frequentes nos instrumentos de qualidade de vida UW-QOL, foram: b152 (Ansiedade - Funções Emocionais), b510 (Funções de ingestão - saliva), b250 (Função gustativa). No questionário BCSQ H&N, as categorias mais frequentes foram: b510 (Funções de ingestão), b310 (Funções da voz) e b152 (Funções emocionais). No questionário OHIP-14, as categorias mais pontuadas foram: b280 (Sensação de dor), b152 (Funções emocionais) e b130 (Funções da energia e dos impulsos - Atividade). Foi observada equivalência entre os domínios dos instrumentos de qualidade de vida e a CIF. Assim, a CIF constitui instrumento útil para avaliar a real situação de saúde do paciente, além dos aspectos clínicos.


The World Health Organization (WHO) defined quality of life as the perception of the individual in his life, in the context of values in which they live and in relation to their goals, expectations, parameters and social relations. The objective of the study was to perform the equivalence of quality of life instruments, University of Washington - Quality of Life (UW-QOL), Brief Core Set Questionnaire - Head and Neck (BCSQ H&N) in patients with diagnosed oral cancer in the League of Oral Neoplasms (LINB); and the Oral Health Impact Profile (OHIP-14) for patients who sought for emergency care, with the International Classification of Functioning, Disability and Health (ICF). Fifty patients from the Oral Neoplasms League and 100 patients from the Urgency Clinic at FOUSP were interviewed. Each instrument's scores were registered and the occurrence frequency of each domain was verified. The most frequent categories of instruments in quality of life UW-QOL were: b152 (Anxiety - Emotional functions), b510 (Ingestion functions - saliva), b250 (Taste function). At BCSQ H&N the most frequent categories were: b510 (Ingestion functions), b310 (Voice functions) and b152 (Emotional functions). In the OHIP-14 questionnaire, the highest scoring categories were: b280 (Sensation of pain), b152 (Emotional functions) and b130 (Energy and drive functions - Activity). Equivalences was observed among the instruments´ domains and ICF. Thus, ICF is a useful tool to perform real evaluations on the patients´ health, adding information to the clinical aspects.

O estudo das células-tronco cancerígenas (CTCs) durante o processo de malignização e no carcinoma epidermóide intra-bucal já instalado é essencial para um melhor entendimento de como essas células participam da formação e manutenção de uma neoplasia. Atualmente, a identificação de células com características de células tronco se dá principalmente através da expressão de marcadores celulares como o ALDH1 e o CD44. A proteína ALDH1 é responsável pela oxidação de aldeídos intracelulares e vem sendo utilizada para o isolamento de CTCs em inúmeros canceres incluindo casos de cabeça e pescoço. A proteína CD44 é uma glicoproteína envolvida na adesão e migração celular, também participa do processo de metástase e já foi associada às CTCs. Nesse trabalho, a expressão dessas proteínas foi analisada em 45 casos de displasias epiteliais e 13 casos de carcinomas epidermóide intra-bucais. As lesões displásicas foram classificadas em casos leves (19), moderados (18) e intensos (8) e foram também divididas em casos de baixo risco (22) e alto risco de transformação maligna (23). A expressão imunohistoquímica para a ALDH1 foi encontrada predominantemente na camada basal em 16 casos de displasias epiteliais e em 7 carcinomas epidermóides, com a marcação difusa pela epitélio neoplásico. A expressão imunohistoquímica de CD44 foi encontrada em 42 displasias epiteliais e em 12 carcinomas epidermóides, sendo que nas displasias, a expressão ocorreu predominantemente na camada basal do epitélio e no carcinoma epidermóide a expressão foi disseminada. Ambos marcadores exibiram aumento de expressão com a evolução do grau das displasias.


The study of cancer stem cells (CTCs) in the process of malignant transformation and intra-oral squamous cell carcinoma already installed is essential for a better understanding of how these cells participate in the formation and maintenance of a neoplasm. Currently, identification of cells with characteristics of stem cells is primarily through the expression of cell markers such as CD44 and ALDH1. The ALDH1 protein is responsible for the oxidation of intracellular aldehydes and has been used for the isolation of CTCs in numerous cancers including head and neck cases. The CD44 protein is a glycoprotein involved in cell adhesion and migration, also participates in the process of metastasis and has been associated with CTCs. In this work, the expression of these proteins was analyzed in 45 cases of epithelial dysplasia and 13 cases of intraoral squamous cell carcinomas. The dysplastic lesions were classified as mild (19), moderate (18) and intense (8) cases and were also divided into low-risk cases (22) and high risk of malignant transformation (23). The immunohistochemical expression for ALDH1 was found predominantly in the basal layer in 16 cases of epithelial dysplasia and squamous cell carcinoma in 7, with diffuse labeling by neoplastic epithelium. Immunohistochemical expression of CD44 was found in 42 epithelial dysplasias and 12 squamous cell carcinomas, and in dysplasias, the expression occurred predominantly in the basal layer of the epithelium and in squamous cell carcinoma expression was widespread. Both markers showed increased expression with the evolution of the degree of dysplasia.
